REGISTRATION FORM QUARRYSIDE PRACTICE

REGISTRATION FORM CHILD UNDER 12 YEARS

To join the practice please complete the NHS Health Board Registration Form and the Quarryside Registration Form.  If you are registering a child please complete the appropriate form and email them to us with photo ID and proof of address to ggc.gp87305clinical@nhs.scot.

In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice. If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.

Free NHS treatment for patients from abroad depends on the length and purpose of your residence in the UK.  Normally you will require to be ordinarily resident in the UK and be able to show that your stay has some degree of permanence and stability. Patients may be asked to see proof of identity, for example photo identity such as your passport or driving licence, and proof of your address such as a recent gas bill or council tax bill.  Visitors from abroad may be asked to show a passport and visa.

Patients are registered with the practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of a GP partner; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis and we will do our best to respect your choice. However, Dr Sim is part time and is not in the practice every day so you may have to wait longer to see her if her clinic is busy.
